As part of our Partner's in Education Program at Memorial Hospital, The Mississippi State Department of Health, The School District, and Memorial Hospital at Gulfport are working together to offer all seniors,18 and under, at Gulfport High School, the opportunity to receive immunizations that will be required fo college admission. This service will be provided free of charpe. We will offer the Measles, Mumps,Rubella (MMR), Tetanus, Meningitis, and the Hepatitis B Vaccine (a series of 3 shots over a 4-6 month
period) to your graduating senior, on March 26,2009. Attached you will find important information on the MMR, Tetanus, Meningitis, and Hepatitis B immunizations. Please read this information carefilly and consult with your child's physician if you are unsure about whether or not your child should receive these vaccines. If you want your child to participate, you will need to sign the attached Vaccine Administration Record and have your child return the form to the school.
If your student is 18 years old h/lshe can sign hisher own consent for immunization. If we do signed Vaccine Administration Record vour child will not be able to participate.
All senior colleges require documentation that in-coming students have had two (2) MMR immunizations. A few gfaduating seniors have had only (1) MMR as that is all that was required when they started school.
Some colleges, but not all require the Meningitis and/or Hepatitis B vaccine. Hepatitis B vaccine is given in a series of three (3) shots, offered over a four to six month period. Your child may start the first shot on
the MHG-Van on the day we are scheduled to be at the school, but understand that you will need to locate the MHG Van for the remaining two shots. A schedule of Van locations (various locations around Harrison County) may be received by calling 867-4246 or logging on to our website: www.gulfportmemorial.com and clicking on Memorial Immunization Van. Remember that all three doses
are needed for full and lasting immunity.
We will only be available on the above scheduled day at your child's school, so it is imperative that the
proper paperwork be completed prior to the MHG Van's arrival. Please return the completed Vaccine Administration Record along with a of their current immunization record to the school office bv March 11.2009. No packets w i l l accepted after this date. Current immunization records can be obtained from the health department or your health care provider if necessary.
Because of scheduling with our school districts, we are unable to make additional visits to the school. If your child is absent on the day that we visit their school, helshe can certainly come to any of the Van locations to get hisher immunizations. Again, no immunizations will be given without a Vaccine
Administration Record signed by the parent or student who is 18 years of age.
We are proud to provide this invaluable service to each of our school districts and hope that you will take advantage of this program. If you have quesions about the immunizations, please contact your physician or feel free to contact Community and Corporate Relations at 867-5210.
